We want to manage your appointment effectively, and we value your time and safety. You can help by printing and completing the relevant patient forms before your arrival.
All new patients must complete a General Registration form. If you have had an exam with us previously, you do not need to fill out this form. Please use our Medical Records Release Request Form to either request your medical records from us, or to grant us permission to acquire your prior exams from an outside facility.
When scheduling your CT examination, our patient scheduling representatives will ask you important questions during pre-screening to ensure safety and accuracy. Also, the CT Patient History Form (haga clic aquí para la versión española) is required for all CTs. If your test is with IV contrast, you must also complete the CT IV Contrast Informed Consent (haga clic aquí para la versión española).
When scheduling your MRI examination, our patient scheduling representatives will ask you important questions during pre-screening to ensure safety and accuracy. Also, the MRI Patient History & Consent Form (haga clic aquí para la versión española) is required for all MRIs. It is very important to complete these forms carefully, as the high magnetic fields in MRI can be dangerous with certain types of implantable devices. Please locate and bring all of your medical device manufacturer cards with you if you have had prior surgeries or device implantations.
Before your mammogram, you will be asked to fill out the Mammogram History Form. It is very important to complete this form accurately for the best quality mammography interpretation by your radiologist.
The DEXA Questionnaire is required before your DEXA bone density exam to ensure proper evaluation and an accurate assessment of your ten-year fracture risk.
High quality PET/CT requires excellent blood sugar control for optimal results. Please review our PET/CT General Preparation Instructions (currently being updated). Depending on the nature of your exam, it may be necessary for you to fill out several of the following forms (not a complete list):
